Pfizer “Sana La Rana” Outreach Project

The Hudson Perinatal Consortium partners with Pfizer to provide outreach to the Hispanic community in Hudson County. Pfizer has built a special equipped outreach vehicle to provide services in the community. These services include:

• Enrollment in NJFamilyCare for clients who qualify for this program.
• For clients ineligible for FamilyCare, advisors help them identify local providers in their community who accept charity care, uncompensated care, managed care and private insurance.
• Nursing staff from the Jersey City Family Health Center provides screenings for cholesterol, glucose, blood pressure and depression. These screenings are designed to educate clients on the importance of seeing a physician to help evaluate their health status. Clients receive a copy of their screening results to take to their physician, and are encouraged to see a doctor for appropriate medical care.
• Consumer group classes on diet, exercise, stress management and various disease states, such as asthma, diabetes, and heart disease, are routinely offered. Hudson Perinatal Consortium also provides breast self-exam information and instruction.
• A partnership between Pfizer, Shop Rite and the Hudson Perinatal Consortium provides a forum to educate the public about health issues. Pfizer provides physicians who conduct a series of lectures at Shop Rite locations throughout Hudson County. The Consortium supplies bilingual outreach workers to provide translation services for the medical personnel.
