Home Visiting Programs

Home visiting is an early intervention strategy featuring the home-based delivery of preventive health care, health education, support services and linkages to existing services for pregnant women, parenting families and children.

Home visits are the core of our successful outreach program. Outreach workers visit clients in their homes offering to help arrange medical appointments and secure social services. Home visits were also provided to help clients enroll in NJ FamilyCare, for breastfeeding support and for breast cancer patients enrolled in the Hudson Breast Health Initiative.
